As an opioid medication it has a powerful effect on the way in which pain signals are sent to the brain. Back Pain
Back pain has many causes, and as such treatment with Tramadol is not universal, either in appropriateness or type. As many back pains are chronic (long term), Tramadol is not suitable in some cases. Sciatica
Sciatica is often treated differently than most back pains as its cause is a more specific one (being that it is the compression of the sciatic nerve, which runs from the lower back to the feet). As such it has a more defined course of treatment. The pain of sciatica can be severe, but it is typically fairly short lived (rarely more than 6 weeks), which makes it particularly well suited for opioid painkillers like Tramadol in conjunction with anti-inflammatory.
Nerve pain of all kinds is extremely painful, so Tramadol immediate release capsules are therefore better suited as there fast working effects offer quick relief. They can be taken every 4-6 hours at a strength that can be titrated (increased) slowly as needed, depending on the severity of the pain and how well the medication is controlling it. This also means the medication can be easily discontinued as the pain subsides to a manageable level in short-term cases. If the condition does not improve after a few weeks, another course of action may be required as well as further investigations as to what is causing the problem.

Tooth Ache (Tooth Pain)
Tooth aches will rarely require an opioid painkiller to manage pain, with Ibuprofen being the most likely course of action. In extreme cases, however, a more powerful painkiller can be prescribed.
While Tramadol in combination with Ibuprofen is an effective painkiller for tooth pain in general, most prefer Codeine combined with a strong anti-inflammatory as long as the issue is short-term. Prolonged acute pain, however, might be better suited to Tramadol as it has less side effects and reduced chance of addiction occurring by comparison.

Migraines
Tramadol use for migraines has been well studied and proven for its efficacy for most symptoms. It is also well suited to the condition as migraines tend to not last longer than a few days, meaning side effects and other risks are reduced significantly.
In cases where pain is particularly severe it is particularly effectual, providing instant relief when taken at even low doses. Restless Leg Syndrome
Restless Leg Syndrome, or Willis Ekbom Disease, is not fully understood regarding its causes, but it is thought to be a disorder of the central nervous system. It is therefore well suited to Tramadol, which specifically targets the central nervous system as a matter of course. It also has an effect on Serotonin and Dopamine levels, which have also been proven to influence Restless Leg Syndrome.
